OSAC Scholarship Application

The Ford ReStart Program is for non-traditional students over the age of 25 who wish to begin or continue on an educational track at college. To be considered for a ReStart scholarship from The Ford Family Foundation, applicants must submit a ReStart Reference Form as well as a complete OSAC scholarship application packet. Applicants must have one reference completed by an employer, colleague, academic advisor, counselor, teacher, church representative, or supervisor in a community activity.  This form must be received no later than March 2, 2009.

Two options for submitting the reference are available.  If the applicant chooses to independently e-mail the reference, see OPTION 1.  If the applicant chooses to supply a paper copy to the reference, see OPTION 2.

OPTION 1: Applicant Downloads a Fillable PDF Version of the ReStart Reference Form

The applicant must:

1. Download the ReStart Reference Form.
2. Complete the applicant portion of the form and save it to the computer.
3. Send an e-mail to the reference with the ReStart Reference Form as an attachment.
4. Ask the reference to mail or e-mail the completed form to The Ford Family Foundation Scholarship Office.

OPTION 2: Applicant Prints the ReStart Reference Form

The applicant must:

1. Download and print the ReStart Reference Form.
2. Complete the applicant portion of the form and give the form to the reference.
3. Ask the reference to mail or fax the completed form to The Ford Family Foundation Scholarship Office.

Remind your reference that the ReStart Reference Form must be received by March 2, 2009 or your application will be considered incomplete.
