OSAC Scholarship Application

The Ford Family Foundation 2009-10 Special Recommendation Form is a supplement to the Oregon Student Assistance Commission (OSAC) Application. It is required if:

• You apply for either the Ford Scholars or Ford Opportunity Program AND
• Your cumulative GPA (unweighted high school or college) is less than 3.00 or your GED score is less than 2650 (or less than 265 for pre-2002 score).

If you apply for either of these programs and you do not meet the minimum academic requirements, you must submit this form as well as a complete OSAC scholarship application packet. You must have one recommendation form completed by an academic advisor, counselor, or teacher. The form must be received no later than March 2, 2009.

Two options for submitting the reference are available.  If the applicant chooses to independently e-mail the reference, see OPTION 1.  If the applicant chooses to supply a paper copy to the reference, see OPTION 2.

OPTION 1: Applicant Downloads a Fillable PDF Version of the Special Recommendation Form

The applicant must:

1. Download the Special Recommendation Form.
2. Complete the applicant portion of the form and save it to the computer.
3. Send an e-mail to the reference with the Special Recommendation Form as an attachment.
4. Ask the reference to mail or e-mail the completed form to The Ford Family Foundation Scholarship Office.

OPTION 2: Applicant Prints the Special Recommendation Form

The applicant must:

1. Download and print the Special Recommendation Form.
2. Complete the applicant portion of the form and give the form to the reference.
3. Ask the reference to mail or fax the completed form to The Ford Family Foundation Scholarship Office.

Remind your reference that the Special Recommendation Form must be received by March 2, 2009 or your application will be considered incomplete.
